We hope to be able to help you send your student to camp in any way we can! This form contains a scholarship request section and a payment plan request section. Please fill out BOTH sections in a timely manner. We will get back to you as soon as possible, but no later than at least two weeks prior to the camp, to discuss how best we can be of assistance! This form can be utilized for 4th & 5th Grade UCYC, Middle School or High School Camp.
The completion of this form DOES NOT reserve your student's camp spot. To reserve your student's camp spot you must complete a camp form and pay the non-refundable $100 deposit.
